CURRENT PATIENTS
For established patients at The Memory Center, you can expect ongoing medical care and access to a variety of support:
Follow-up medical visits
Regular follow-up clinic visits with The Memory Center care team to monitor progression of condition, adjust medications as needed
Ongoing Social Work Support for Patients and Family / Care Partners
Personalized social work support and resources to manage day-to-day life and its necessary transitions over time. Includes communication strategies coaching through memory loss, developing a short- and long-term care plan for patient and care partner(s)/family members, and ways to explore in-home care or community residential living options.
Support & Quality of Life Programs
The Memory Center hosts a variety of support and quality of life programs:
- Alzheimer’s Association Caregiver Support Group
- ‘Art Is’ Therapeutics program, hosted by GoldMind Arts and Aging
- Annual Chicago River Cruise: A Dementia Friendly event
- SHARE Network Chicago Events (e.g. Artful Aging at the Hyde Park Art Center, educational events at Mather’s More than a Cafe, local caregiver support groups)
